Plan Ahead:
When scheduling your appointments,write it in your calendar, or create a reminder in your phone to make sure thatyou don’t miss your appointment. Let us know if you need transportation to andfrom your appointments at least a day in advance.
Prior to your first visit with us, please make sure of the following:
- Bringyour Insurance forms and insurance cards with you to your appointment
- Ifyour insurance requires a co-payment, please note that it is due at the time ofyour appointment.
- Medicalrecords and/or radiology films from your physician have been requested andaccompany you to your appointment.
- Youhave a list of any medications that you are currently taking.
- Youhave a list of questions that you may want to ask one of our therapists
- Ifyou are being treated for a workers’ compensation injury, please inform ourreceptionist, as you will need to get prior approval before your appointmentdate.
- Ifyou need to cancel or reschedule your appointment, please call our office tolet us know as soon as possible.
- Bringa Picture ID with you.
